Everything you need to register your child, complete a medical release, apply to volunteer, or review our child safety policies is here in one place.

For Families

  • Children’s Ministry Registration Form: Register your child to be added to our Sunday check-in system. This form is required for participation in nursery, Sunday School, Children’s Church, and most Children’s Ministry events.
  • One & Done Medical Release Form: This form covers your child for all Children’s Ministry events during the program year — complete it once, and you’re set for the entire year. Required for all events and off-campus activities.

For Volunteers

  • Volunteer Application: This form starts the process of serving with Children’s Ministry.
  • Darkness to Light Training Signup: Darkness to Light (Stewards of Children) is a nationally recognized child sexual abuse prevention training required for all Children’s Ministry volunteers. Completion is required before your first volunteer shift.

Policies

  • Safe Environment Policy: Oak Grove UMC is committed to providing a safe environment for every child in our care. Our Safe Environment Policy outlines the standards and procedures that all staff and volunteers are required to follow, including background checks, two-adult rules, and reporting obligations.
